Persistent Cough? Know When It’s Time to Get It Checked

A cough is a common symptom that people experience at some point. In many cases, it settles within a few days or improves as a cold or flu clears. However, when a cough takes several weeks to clear, or starts affecting your daily life, it is worth getting it checked.

A persistent cough should not always be dismissed as a lingering cold or treated only with home remedies. It can sometimes be linked to an underlying respiratory condition that needs proper evaluation and treatment.

When should a cough be considered serious?

Coughing is a natural reflex that helps clear mucus, irritants and foreign particles from the airways. A cough that does not settle on its own may mean that something is continuing to irritate the respiratory tract.

Depending on the cause, a persistent cough may be accompanied by:

  • Sore or irritated throat
  • Wheezing
  • Dry or irritating cough
  • Mucus or phlegm
  • Difficulty taking a deep breath
  • A feeling of congestion in the chest

How long is too long for a cough?

The duration can provide an important clue:

  • Less than 3 weeks: Usually considered an acute cough
  • 3 to 8 weeks: Generally described as a subacute cough
  • More than 8 weeks: Considered a chronic cough

It is important to evaluate the cough that continues beyond the expected recovery period, especially if it is getting worse.

What can cause a persistent cough?

There are several possible reasons why a cough may continue. Common causes include:

  • Smoking
  • Pneumonia
  • Tuberculosis (TB)
  • Allergies
  • Certain medications
  • Viral infections that cause a cough to linger after other symptoms have improved
  • Bronchiectasis
  • Asthma
  • Acid reflux or GERD
  • Long-term exposure to pollution, dust, fumes or chemicals

Lung cancer is a less common cause, but a persistent cough should still be investigated when other warning signs or risk factors are present.

Warning signs of cough that you should not ignore!

Certain symptoms should prompt you to seek medical advice.

  • Shortness of breath: If the cough is accompanied by breathlessness, chest tightness, or wheezing, it could be a sign of asthma, pneumonia, COPD, or even heart-related conditions, indicating the need for early medical intervention.

  • Cough that lasts for several weeks: Despite medical intervention and home care, if your cough refuses to go away, then it may indicate chronic bronchitis, asthma or allergies, acid reflux, tuberculosis or other lung conditions. If these conditions persist, seek medical attention immediately.

  • Coughing up blood: Blood in the sputum should not be ignored, even if the amount is small. It can occasionally result from irritation caused by persistent coughing or by infections or conditions such as tuberculosis, pneumonia, bronchiectasis or lung cancer.

  • Chest pain: Chest pain that occurs with a persistent cough deserves attention, particularly when the pain is severe, ongoing or associated with breathing difficulty. It may occur because of a respiratory infection, strain from repeated coughing or inflammation around the lungs, among other causes.

  • Foul-smelling, thick or coloured sputum: The yellow, green or foul-smelling mucus may alert you to the risk of bacterial infection. Even large amounts of sputum may indicate chronic lung health issues that require medical intervention and investigations.

  • Cough with fever, night chills and weight loss: A prolonged cough accompanied by fever, night chills, and weight loss can be a warning sign of an underlying infection or other medical condition. Tuberculosis is one such possible cause.

  • Long-term exposure to smoke, dust or pollution: People who smoke regularly or are exposed to industrial dust, biomass smoke or air pollution should take a persistent cough seriously. This may also be an early warning sign of lung cancer or lung disease.

How can a chronic cough be detected?

The evaluation depends on your symptoms, medical history and risk factors. Your healthcare provider may suggest various tests to identify the cause, such as:

  • Physical examination: Your doctor may listen to your lungs with a stethoscope for sounds such as wheezing or crackling and assess your breathing.

  • Chest X-ray: It can help identify certain lung abnormalities, including pneumonia and some findings associated with tuberculosis.

  • Lung function tests: These tests assess how well your lungs work and can help detect conditions like asthma or COPD.

  • Blood tests: Depending on your symptoms, blood tests may be used to look for signs of infection, inflammation or other abnormalities.

  • Sputum tests: If you are producing phlegm, a sample may be examined for certain infections, including tuberculosis.

  • Allergy testing: If allergies are suspected, testing can help identify triggers.

  • Reflux evaluation: If acid reflux or GERD is suspected, your doctor may assess whether reflux could be contributing to the cough.

How can I reduce the risk of cough?

Some simple measures can help protect your respiratory health and reduce exposure to common cough triggers:

  • Avoid smoking and secondhand smoke.
  • Wear a suitable mask when exposed to dust or high levels of pollution.
  • Drink enough fluids throughout the day.
  • Keep allergies under control.
  • Avoid known triggers that worsen your cough.
  • Stay up to date with recommended vaccinations.
  • Seek medical advice if a cough does not improve as expected.

When to consult a doctor?

When your cough becomes severe, with signs like trouble breathing, coughing up large amounts of blood, sudden confusion or dizziness, or chest pain spreading to your arm or jaw, seek timely medical attention.

You should speak to your doctor if your cough is persistent, worsening, or accompanied by other concerning symptoms to identify the underlying cause and receive appropriate treatment. Do not ignore a cough that keeps coming back or interferes with your daily activities.

Finding the cause early can help ensure appropriate treatment and reduce the risk of complications.

FAQs

1. What are the common causes of a persistent cough?

Allergies, asthma, infections, acid reflux, smoking, or other lung conditions can cause a persistent cough. Hence, identifying the cause is important for choosing the right treatment on time.

2. Why is my cough not stopping?

A cough may persist due to infections, allergies, asthma, acid reflux, or post-nasal drip.

If it lasts for several weeks or gets worse, consult a doctor to identify the underlying cause.

3. Can a persistent cough be treated?

Yes, treatment depends on the cause and may include medicines or lifestyle changes. A doctor can recommend the right treatment after evaluating your symptoms.
